- A Co-operative is a member-owned business with membership open to those who use its services. Democratically controlled and operated on a nonprofit basis, a co-operative returns any margins or profits to members on the basis of patronage.
- The aim of the Springville Co-operative Telephone Association is to make telephone, television, and internet service available to its members at the lowest cost consistent with sound economy and good management.
- Terms and conditions following application of service from Springville Co-operative Telephone Association:
- The applicant will pay to the Co-operative the sum of $20 and upon acceptance by the Cooperative will constitute the applicant's share of voting common stock. Additional deposits may be required. When the applicant terminates all service with the Co-operative, the $20 shall be refunded providing nothing is due and owing to the Cooperative at that time.
- The applicant will, when telephone service becomes available, purchase from the Co-operative all telephone service used on the premises described below and will pay therefore monthly rates which will be fixed by the Board of Directors of the Co-operative. The applicant will pay a monthly bill governed by type of service given.
- The applicant will grant the Co-operative and its subsidiaries a right of way easement to construct, operate and maintain telecommunications and television lines, cables or systems on the applicants land at the address indicated above. The easement shall be granted for the purpose of serving the applicant or other members of the Co-operative when the easement is necessary to provide telecommunication and television services in the safest and most efficient and economical way possible. If the telecommunications and television facilities are constructed underground, the Co-operative may record the easement for the future benefit of its members. An easement shall be of perpetual duration unless other arrangements are agreed upon by the Co-operative and the applicant.
- The applicant will comply with and be bound by the provisions of the Articles of Incorporation and By-Laws of the Co-operative, and such rules and regulations as may, from time to time, be adopted by the Co-operative.
